Overall aim of the program in which the role exists:
Strategically plan, develop and evaluate projects to enhance nursing practice in private and voluntary nursing homes The practice development facilitator will create a culture to support Nursing Home practitioners through research, continuing professional development and self evaluation to develop a culture of evidence based practice in all aspects of Care of Older Persons within the Nursing Home Sector.
Essential
• Be on the Register of nurses maintained by An Bord Altranais.
• Have a minimum of 5 years post registration experience of which 3 must be in Care of the Older Person
• Evidence and Expertise in Practice Development within Nursing
Desirable
• Management Qualification and Experience in a managerial role
• Experience in Practice Development and Project Management
• Degree / Masters in Nursing /Health related subject
• Post Graduate Diploma in Gerontological Nursing
• Experience/knowledge of working in a nursing home
• Knowledge of HIQA National Quality Standards for Residential Care Settings for Older People
• Knowledge of Health Act 2007, Health Act 2007 (Care and Welfare of Residents in Designated Centres for Older People Regulations 2009, Health Act 2007 (Registration of Designated Centres for Older People) Regulations 2009
Role The appointee will work in conjunction with the nursing committee to raise and set standards of quality of care by creating a culture of evidence based practice. Strategically plan, develop and evaluate projects to enhance nursing practice in care of the elderly residential settings provided. This role requires a combined knowledge of clinical practice, project management, service management, continuing education and professional development Liaison with all organizations with an interest in care of the older person including providers of training
Principal Responsibilities Older Persons Services Nursing Practice
Review care of the older person nursing practice through audit and evaluation. Support Directors of Nursing in performing a lead role in professional development in nursing care of older person practice and education. Disseminate professional and clinical knowledge. Endorse and enable life long learning through professional development
Quality initiatives and Standard setting
Develop systems, which agree, monitor and improve standards of care. Enable the continued development of clinical practice guidelines, policies and protocols. Facilitate and support care of the older persons nursing through an accreditation process
Continuing Education
Plan, co-ordinate and evaluate continuing nurse education programmes specific to care of older persons. Organise and run educational sessions, workshops, study days and seminars Establish a framework for development of clinical competencies. Advanced Practice
Develop nursing practice by actively supporting and developing advanced clinical practice using Scope of Practice framework. Support the role of advanced clinical practitioner in care of the older person nursing and enable development and audit of this role.
Evaluation of Service
Develop systems that continuously evaluate the services' potential to meet consumer needs.
Risk Management Collaborate with the risk management system, contributing professional nursing expertise. Develop and implement education and development programmes to support the elimination of risks.
Research Support nursing participation in research. Be conversant with current trends in innovative nursing practice in older person's services.
